Scheduling your appointment:

  • At Carolina Pediatric Surgery, we strive to make your visit as pleasant and convenient as possible.

  • All appointments should be scheduled beforehand. Walk ins are welcome but will be accommodated based on our doctor's and staff availability.

  • Remember that most appointments require a referral letter sent by your primary care physician to our office. Some insurance carriers require pre-authorization prior to the visit. Please check with your insurance carrier prior to coming to your visit.

  • All patients 18 and younger should be accompanied by one of the parents/guardians at the time of the visit. Failure to do so will result in your appointment being rescheduled.

  • Please arrive to the appointment 15 minutes before the scheduled time to avoid cancellation. This is a must if you are a new patient.

  • We try as much as possible to be mindful of your time as well as the time of other patients being seen in our clinic. If you are more than 15 minutes late, we will check with our doctor to see if we can accommodate the delay. However, there is no guarantee you will be seen the same day and you risk your appointment being rescheduled.

  • All no-shows will be charged a fee of $35 unless the appointment is cancelled 24 hours before the scheduled time.

  • Please check our "Forms" section and print and fill all required forms prior to your appointment. This will help us get you checked in faster and make efficient use of your time. This is very important for new patients. Please check the "New Patients" forms and print and fill all pertinent forms prior to the appointment. Make sure you chose the right "New Patient Form" as there is one for adults and another one for children/adolescents less than 18 years of age.

  • We are trying to go paperless to minimize handling paperwork and reduce infection transmission. It would be ideal to fill and sign the forms electronically beforehand and email them to us at info@carolinapediatricsurgery.com. We appreciate your help with this.

You can use the free version of Acrobat Reader DC to fill and sign
