YOUR OFFICE VISIT

A child’s visit to the doctor can be challenging for patients and families. Our team is trained in caring for young patients, and our goal is to make your visit as stress-free as possible.

Preparing for your visit:

  • If your child is a new patient, please print and fill the new patient forms available on our website . Please complete the forms and bring them to your visit. This will greatly expedite the process of registration and your office visit.

  • Please arrive 10-15 minutes early.

Be sure to bring:

  • Identification, such as a driver's license.

  • Insurance card.

  • Proof of legal guardianship or medical power of attorney if anyone other than the custodial parent is bringing the child to the appointment.

  • List of your child's medications.

  • Any medical records relating to your child's condition.

During your visit:

  • A nurse will review the packet with you and may ask for additional information.

  • The nurse will weigh your child and take vitals such as blood pressure and temperature.

  • You will then meet our surgeon, who will conduct an examination, carefully explain the necessary testing and treatment, and answer all your questions.

  • Minor surgical procedures may be performed in the office.

  • If your child is scheduled for surgery, you will be scheduled for a surgery date and will be given instructions on where the surgery will take place.

  • Sometimes, we will not be able to provide you with the exact day of surgery at the time of your visit. This is usually in order to obtain insurance verification and approval for the procedure by your insurance carrier. In this case, we will call you as soon as we get approval to coordinate the date of surgery. PLEASE MAKE SURE WE HAVE A GOOD WORKING PHONE NUMBER TO REACH YOU.
